Hospice care is our dedicated focus on providing comfort, solace, and a nurturing environment to individuals diagnosed by a doctor with a prognosis of six months or fewer. We ensure that every individual experience dignity, love, and peace during their journey.

 

Initiating the journey towards hospice care is simple. We collaborate with the patient’s physician, ensuring a shared understanding of the appropriateness and benefits of hospice care. To commence this transformative experience, the patient will be invited to sign consent forms, allowing us to provide an array of compassionate services tailored to their unique needs.

 

We strive for empowering our patients to make choices aligned with their wishes. While a DNR order is not a requirement for hospice care, we take the time to educate patients and their families on the significance of advance directives. Our dedicated team provides comprehensive information and guidance, ensuring that each person’s care plan reflects their values and desires.

 

Hospice care knows no boundaries when it comes to compassion and inclusivity. We extend our support to patients facing a range of diagnoses, including Alzheimer’s Disease, Dementia, Parkinson’s, Cancer, Cardiovascular Disease, Renal Disease, Neurological Diseases, Pulmonary Disease, COPD, HIV, and Liver Disease. Our goal is to provide comfort and understanding, regardless of the condition.

 

Hospice care is generally funded through various sources i.e., government programs and private health insurance. The primary option  is Medicare. Medicaid also covers hospice services with eligibility  requirements varying by state. 

 

Our compassionate care extends to various settings, including the comfort of the patient’s own home, Assisted Living facilities, Nursing homes, Short-term inpatient facilities, and Senior Living communities. We adapt to meet the unique needs and preferences of each individual, creating a serene and supportive environment wherever they call home.

 

At Cozy Transitional Hospice Care, we believe in the power of presence and support. While continuous care is provided in certain circumstances, we also acknowledge the significance of family involvement and personal connections. Our dedicated team of professionals is available to provide support, ensuring that you never walk this journey alone.
Hospice provides continuous care in some circumstances, but other circumstances would require families to supplement the care of hospice. Cozy Transitional Hospice care provides 24/7 on-call nursing.

What types of Hospice Care are available?

This level of care is offered during non-crisis situations for the patient. It can be offered in different settings, including the patient’s residence, a skilled nursing, or an assisted living facility. Our compassionate team ensures that the patient receives the necessary support and comfort during this time.

 

Patients who require ongoing assistance can receive hospice aide and homemaker services continuously. This level of care is provided during brief periods of crisis to maintain the patient’s well-being at home. Our dedicated team is there to offer support and help manage any challenging situations that may arise.

 

Caregivers also need time to rest and recharge. Inpatient respite care allows the patient to receive temporary care in an approved facility for up to 5 consecutive days. This provides the caregiver with a much-needed break, knowing that their loved one is in safe and capable hands.

 
 

In some cases, pain control or symptom management may require more intensive support. Our general inpatient care is provided in a specialized facility to address acute or chronic symptoms that cannot be effectively managed in other settings. This ensures that the patient’s comfort and well-being are prioritized.
